How Do I....
Obtain A Repeat Prescription?
Repeat prescriptions will be issued at the doctor's discretion and are normally for patients on long-term treatment. Requests for prescriptions can be made in writing or by calling in at the
surgery from 8.00am to 1.00pm and 3.00 to 6.00pm Monday to Friday.
If you would like the prescription posted to you please supply an SAE. If you wish the pharmacy to collect and deliver, please indicate this on your request. To avoid mistakes, we are unable to
accept telephone requests for repeat prescriptions. We are unable to take orders or issue prescriptions at weekends, public holidays or out of normal surgery hours.
Please allow two working days before collection of repeat prescriptions and make allowances for weekends and public holidays. Where possible give exact drug names when ordering.
THIS FORM BELOW IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
